Fantasy and Science Fiction by Jack Vance contains a brief bibliography of Jack Vance and a checklist of his work, excerpted from a work in progress of Daniel Levack. That work culminated in 'Fantasms: A Jack Vance Bibliograph', by Daniel Levack and Tim Underwood (1978).
The pamhlet was given to subscribers of 'The Eyes of the Overworld', published by Underwood-Miller in 1977 in a limited edition of 1500 copies. This suggest that a maximum of 1500 copies, but most likely less, of the pamhlet were made.
On the front end paper and the back end paper two illustrations by Virgil Finlay, one illustration by Jack Gaughan, one illustration by George Barr, and one illustration by Stephen Fabian are depicted. The cover art by Edd Cartier is taken from 'The Potters of Firsk'.
